• Relevant company background
• Number of employees
• Drawings of the worksite
• Location of hazard(s)
• Workers impacted by the hazard
• Pictures and/or video of the hazard(s) (blur coworkers' identifying features)
• Testimony from workers about the hazard

• When & Where?
• Where do workers get injured?
• When do workers get injured?
• Which shifts?
• How many people have been injured?
• Is it related to job tasks?
Employer Background
• Does the employer know or have reason to know?
• Have there been complaints to the employer?
• How did the employer respond?
• Did anything change?
Documentation
• When and where did the incidents occur?
• Is the safety problem documented? (i.e. accident report, doctor’s visit record, photos, notes from coworkers, Voice of the Associate/MyVoice, etc.)
Impact & Patterns
• Who’s impacted most by hazards?
• Are there common concerns or pains among certain workers?
